Insurance and Payment
As of July 1, 2021, we will no longer bill insurance for appointments or be in-network with any insurance providers. All patients will be required to pay for their visit at the time of service and we will have set, flat rate prices for each appointment type.
New Patient Visit, 50-60 minutes, $324, includes $50 deposit paid at time of scheduling
Follow-up Office Visit, 20-30 minutes, $144
Annual Physical Visit, 20-30 minutes, $144-$184
Extended Annual Physical Visit, 50-60 minutes, $223
Telephone/Virtual Visit, 20-30 minutes, $144

We will provide a "superbill" at checkout for each visit, which you can use to submit the cost of the visit to your out-of-network deductible or to a health sharing plan. Because individual coverage varies, we cannot guarantee if your insurance will cover any of your visit costs.
We accept Health Savings cards, but we do not accept Care Credit. Payment for your visit is due in full at check-in.

*If you have Medicare, you will not be able to submit your visit for reimbursement. You will need to read and sign our Medicare Opt-Out agreement before you can be seen. The agreement applies only to Lisa J. Roberts's practice, Femme Care Inc. and does not affect your benefits with other providers.
What to expect
Bring your insurance card and a photo ID to your first visit. Plan to arrive 15 minutes early to complete registration and consent paperwork. If you would prefer to complete the paperwork ahead of time, you can print and complete the forms on this page and bring them with you to your appointment. (The Medicare Opt-Out Agreement is only for patients who have Medicare; it is required for all patients who have Medicare as their primary or secondary insurance .)*
For your first appointment, plan for about an hour and a half which includes check-in, time with Lisa, and check out. When you arrive, our office staff will register you and then check your weight, height and blood pressure. Next you'll meet with Lisa. During your time with her you will discuss your health history and any health concerns you would like to work on. You will have a physical exam, including a breast and pelvic exam. You may or may not need a pap smear, but all first appointments with Lisa will include a complete physical exam unless you are referred for a consultation. Lisa will analyze your symptoms and history to determine appropriate bloodwork and/or saliva testing.
- Bloodwork is done at the lab of your choice and can be submitted to your insurance. We provide the lab order form, which you take with you to the lab to have your blood drawn. The results are sent to our office and Lisa will discuss the results with you and make her recommendations at your follow-up appointment.
- Saliva testing is done by ZRT Laboratory and is not billable to your insurance. You purchase the test kit in our office, collect the saliva samples at home, then mail the kit to the lab in Oregon. The results are sent to our office and Lisa will discuss the results with you and make her recommendations at your follow-up appointment.
Scheduling an Appointment
A $50 new patient account set-up fee is required at the time that you schedule your appointment. This fee is non-refundable for any reason if you cancel or miss your appointment. You may reschedule your appointment, but you must provide 2 business days' notice or you will need to pay the fee again to reschedule. New patient appointments are generally available on:
- Mondays at 9:30 am and 2:00 pm
- Tuesdays at 10:00 am and 3:30 pm
- Wednesdays at 9:30 am and 2:00 pm
- Thursdays at 10:00 am
- Fridays at 9:30 am and 2:00 pm
